How do you ask for a raise without it being awkward?

I have been at my company for 2 years, my performance reviews are excellent, and I know I am being paid below market rate. I want to ask for a raise but every time I think about the conversation I get anxious and back out. How do you ask for a raise confidently?

Come prepared with three things: your market rate data, a specific list of your contributions and wins, and the exact number you are asking for. Vague requests get vague answers. 'Based on my research and performance I'd like to discuss a salary of X' is a starting point.

The awkwardness is in your head more than in reality. Managers expect these conversations. What makes it awkward is being unprepared or apologetic. Enter the conversation as a professional discussing business, not as someone asking for a favor.
